CHICAGO MED kicked off its fourth season with Will (Nick Gehlfuss) and Natalie’s (Torrey DeVitto) engagement—and it’s been nothing but trouble for the duo ever since.

Almost immediately after, Will (accidentally) became entangled with an undercover operation after treating his old friend Ray Burke (Dennis Cockrum). And while having to keep that from Natalie has caused a rift between the couple—and has become increasingly dangerous for Will as Ray’s health declines—“it all comes to a head [in the fall finale],” executive producer Diane Frolov teases. “And there’s a very big surprise.”

On the surface, the wedding day starts off well. “Maggie is the maid of honor and Jay Halstead is the best man,” executive producer Andrew Schneider previews. Adds Frolov: “And little Owen is the ring bearer.”

But when Will gets another emergency call to take care of Ray, “he’s torn,” Schneider shares. However, another member of the ED is looped into what’s going: Connor’s (Colin Donnell) patient has a tie to the case, and Jay (Jesse Lee Soffer) “has to read him in.”

Natalie, however, is left waiting for her groom to start their wedding festivities. “She’s just in agony because she doesn’t know what’s happening,” Schneider says. “She’s completely in the dark.”

Will and Natalie won’t be the only couple facing a hardship: Ethan (Brian Tee) is back with Vicki (Cynthia Addai-Robinson), but his relationship with recent ex, April (Yaya DaCosta), remains complicated.

“Vicki is on the scene and Ethan has started a relationship with her, but it’s very tough, because he still has very strong feelings for April,” Schneider says.

“And he and April share a very emotional case together [in the episode],” Frolov adds. “There’s a lot of conflict for Ethan in the episode.”

Will there at least be a wedding ceremony to lighten things up before the fall finale concludes? “We’ll be spending a lot of time at the church,” Schneider teases.
